Dear Applicant,
Enclosed please find an application for the Delaware County
Single Point of Access (SPOA) which is the access point to
obtain Care Management and/or Residential Services for
Adults, aged 18 years and over who are diagnosed with a
Severe and Persistent Mental Illness (SPMI).
The application needs to be completed and submitted with a
copy of a recent (within 6 months) Comprehensive Assessment
and/or Psychiatric Assessment. Also enclosed are two
Authorization for Release of Information consent forms,
giving consent to share the applicant’s information with the
Delaware County SPOA Committee. Applications can be faxed
(607-832-6082) or mailed.
The application is reviewed by the committee and if eligible
for services, the client is assigned to a service provider.
Both the applicant and the referral source are notified of
the disposition of the request for services.
Applicants are invited to attend the meeting to share their
goals and service needs. The SPOA meetings are held on the
second Tuesday of each month at 9:30am.
